About The Position

Continuum Hospice is seeking a dynamic and dedicated nurse to join our team as a LVN - Patient Care Advocate. If you have a background as a hospice clinician, a strong interest in expanding your career into marketing and sales, and are passionate about our hospice mission, we invite you to consider this exciting opportunity. As a LVN - Patient Care Advocate, you will support and partner with current Liaisons to identify new referral sources and maintain ongoing relationships to increase program census and preference for hospice services. LVN - Patient Care Advocates use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our Patient Care Advocates are hospice experts and serve as a resource to our Liaisons and referral sources to ensure extraordinary patient and customer service. Requirements

  • Current unencumbered license as a licensed vocational nurse (LVN) or licensed practical nurse (LPN).
  • Minimum 1 year of experience working directly in hospice required.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Nice To Haves

  • 2-3 years' experience preferred.
  • Prior customer service or sales experience strongly preferred.

Responsibilities

  • Collaborate with our Liaisons to gather crucial patient clinical information, provide admissions support, and actively participate in community events.
  • Join our Liaisons in brand awareness campaigns to make our program the preferred hospice provider in the service area, fulfilling our mission and achieving agency growth and census management goals.
  • Conduct initial meetings with patients and their families, introducing them to the hospice service and our compassionate philosophy, and obtain consent for their care.
  • Ensure compliance with regulatory standards through meticulous record-keeping and prompt and accurate submission of marketing reports.
  • Identify key accounts and help shape our strategy by communicating strengths and weaknesses to our Liaisons and Regional Sales Director.
