The primary goal of this position is to develop business partnerships to generate hospice referral growth through effective sales contacts, calls, and presentations. The Patient Care Advocate will serve as the agency's staff representative with physicians, facilities, discharge planners, and community agencies, taking responsibility for the overall development and referral growth within their assigned territory. The role involves strategic planning, relationship building with healthcare providers, and acting as a liaison to communicate information about services. Additionally, the advocate will meet with patients and families to discuss home care services and manage expectations.
