About The Position

Carlton Senior Living is hiring an LVN to provide care to residents in Assisted Living and/or Memory Care. The role provides direct care, administers medications and treatments, monitors residents’ health, and supports individualized care planning in accordance with physician orders, state regulations, licensure, and community standards. The schedule is on-call with open availability. The stated pay range is $37–$37.

Requirements

Must be at least 18 years old, per Title 22 requirements.
Current nursing license in good standing and free of disciplinary action.

Nice To Haves

At least one year of experience is preferred but not required.

Responsibilities

Provide direct care and administer medications and treatments to residents in Assisted Living and/or Memory Care in accordance with physician orders and state regulations.
Monitor residents’ health conditions, vital signs, and behavior, and promptly and accurately document and communicate changes to physicians, families, and team members.
Support the development and ongoing updates of residents’ Personal Service Plans, coordinating care with interdisciplinary teams and external providers.
Perform medical tasks such as wound care, catheter care, oxygen administration, and injections as permitted by licensure and community standards.
Supervise medication administration, including transcriptions, narcotic counts, and proper storage and disposal, ensuring compliance with policy.
Respond to emergencies, manage family and resident concerns professionally, and maintain thorough shift documentation for quality and compliance.
May assist residents with activities of daily living, including continence care, showering, grooming, transfer assistance, and meal assistance.
Support safety protocols, emergency plans, and elopement prevention practices.
Foster a warm, collaborative atmosphere with residents, families, and team members.
