About The Position

The Licensed Vocational Nurse (LVN) assesses clients and provides nursing care in accordance with physicians’ orders and treatment objectives and as instructed by supervisor. The LPT works under the supervision of an RN in a substance use detox subacute hospital. Both roles involve assessing patients/clients, providing nursing care, monitoring vital signs, administering medications, documenting progress, and working as part of a treatment team. Responsibilities include managing emergencies, maintaining patient safety, adhering to policies and procedures, and ensuring client confidentiality.

Experience level: Experienced
Must be able to successfully complete a medication test.
Must be able to perform CPR and/or first aid.

Responsibilities

Report and record any changes observed in client condition or behavior, including unusual incidents.
Identify, report and assist with the management of emergencies.
Take and record accurate vital signs and perform urinalysis testing.
Chart routine information e.g. vital signs, client progress and daily activities.
Make rounds every 30 minutes for 72 hours on withdrawal management clients, document and record withdrawal symptoms.
Observe and document client self- administration of medications.
Administer anti- overdose medications as needed.
Document client treatment progress.
Conduct medication audits and destroy outdated medications per policy.
Interact with clients to support their participation in the program.
May provide educational sessions or other groups for clients.
Assist with orientation, admission, transfer, and discharge of clients.
Provide clerical support as directed by nursing staff.
Keep charts in order with all forms properly identified.
Follow directions of physicians and registered nurses.
May be required to successfully complete a medication test.
Observe universal precautions, including correct handling of blood borne pathogens, storage of lab specimens, and disposal of infectious wastes.
Remain calm while performing job duties.
Assess and report client behavioral changes.
Identify a crisis.
Demonstrate appropriate techniques for interventions, including AMA, behavioral issues, suicide threat, code green, and contraband on the unit.
Make presentations and facilitate educational groups & sessions as needed.
Work as a member of the treatment team, especially in carrying out the nursing care plan and treatment plan.

May order inventory, and/or stock supplies in the unit.
May be required to assist clients with self-administration of PRN and other medications as ordered by physician.
Courteously answer phones, appropriately directing calls and taking messages.
Make phone calls for clients as approved by the nurse in charge.
Handle clients’ personal belongings as directed.
Run errands within the facility and respond to requests for assistance from the nurse in charge, the DON, and other authorized staff.
Attend meetings and participates in committees as assigned.
Assist in the assessment, planning, implementation, and evaluation of the nursing care plan.
Audit medication room and destroy medications as needed per policy.
Recognize the symptoms associated with chemical dependency and detoxification.
Monitor client care and treatment.
Know and understand withdrawal management.
Participate in nursing assessments.
Adhere to professional standards.
Maintain health and safety standards of facility.
Assess and document patient treatment progress, medical or behavioral changes, and unusual incidents.
Monitor patient management and treatment.
May be assigned to provide continual observation of confused patients.
Safely and accurately administer medications.
