About The Position

At Uvalde Memorial Hospital, hospice care is about helping patients and families experience comfort, dignity, and compassionate support during one of life’s most meaningful journeys. We are looking for a caring and dependable Hospice Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes and support families throughout the end-of-life experience. In this role, you’ll work under the direction of a Registered Nurse or Physician and alongside an interdisciplinary hospice team. Your clinical skills, compassion, and ability to build meaningful connections will help ensure each patient receives individualized care focused on comfort, dignity, and quality of life.

Requirements

  • Current Licensed Vocational Nurse (LVN) license in the State of Texas required.
  • Current BLS for Healthcare Providers certification required.
  • Valid driver’s license, current automobile insurance, and reliable transportation required.
  • Compassionate and professional communication skills with patients, families, and members of the healthcare team.
  • Strong organizational skills with the ability to manage multiple priorities.
  • Proficiency with Microsoft Office, Outlook, and electronic documentation.
  • A dependable, patient-centered professional who understands the importance of comfort, dignity, and compassionate end-of-life care.

Nice To Haves

  • One year of LVN experience preferred.
  • Previous hospice experience preferred.

Responsibilities

  • Provide compassionate palliative nursing care in patients’ homes according to the established plan of care and within the LVN scope of practice.
  • Monitor patients’ conditions and assist with pain and symptom management.
  • Recognize and promptly report changes in a patient’s condition or response to treatment to the supervising RN.
  • Provide care that supports the physical, emotional, and psychosocial needs of patients and their families.
  • Communicate with RN Case Managers following patient visits to support continuity of care.
  • Collaborate with physicians, nurses, and other members of the interdisciplinary hospice team.
  • Maintain accurate, thorough, and timely patient care documentation.
  • Participate in interdisciplinary meetings, in-service education, and performance improvement activities.
  • Help create a safe, respectful, and supportive environment for patients and families.
  • Participate in on-call responsibilities as assigned.
