Hospice - PRN - Licensed Vocational Nurse (LVN)

About The Position
We are seeking a skilled and compassionate Hospice LVN to provide nursing care to patients in their homes under the direction of a registered nurse or physician. The position covers Rowlett, Rockwall, and Forney, Texas (75089). The ideal candidate has hospice care experience, excellent communication skills, and a passion for helping others. Wound care is required 2–3 times per week.
Requirements
- Current LVN license in the state of Texas.
- Minimum of one year of experience in hospice care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong critical-thinking and problem-solving skills.
- Valid driver's license and reliable transportation.
- Ability to pass a background check and drug screening.
- Wound care 2–3 times per week.
Responsibilities
- Assess patients' health status and develop care plans.
- Administer medications and treatments as prescribed by a physician.
- Monitor patients' vital signs and report changes to the RN or physician.
- Provide wound care and other nursing interventions as needed.
- Document patient care and progress in electronic medical records.
- Communicate with patients, families, and other healthcare providers as needed.
- Participate in interdisciplinary team meetings and case conferences.
- Adhere to state and federal regulations and agency policies and procedures.
