LVN Home Care Nurse | Conroe, TX

As an LVN Home Care Nurse, you'll provide personalized, one-on-one care that helps clients remain safe, comfortable, and independent in their own homes. In this role, you'll collaborate with physicians, patients, families, and care teams to deliver individualized nursing care. You'll assess patient conditions, administer medications and treatments, monitor progress, educate patients and caregivers, and communicate clinical updates to ensure the highest standard of care.
Requirements
- Active Texas nursing license.
- Current in-person CPR/BLS certification.
- Valid Texas driver's license.
- Current automobile insurance.
- Recent TB test or chest X-ray.
- At least one year of nursing experience in home health, a healthcare facility, or a clinical setting.
- Commitment to delivering exceptional patient-centered care.
