About The Position

Under the administrative direction of the Center Director, the Home Care Coordinator (HCC) oversees the day-to-day scheduling, quality, and documentation of the Home Care Services program. The HCC serves as the designated Interdisciplinary Team (IDT) member representing home care, per 42 CFR § 460.102, and directs and supervises caregivers and home care scheduler. HCC assists leadership in policy formation, program development, and implementation. HCC will be responsible for conducting home visits and home safety/care assessments and coordinates the plan of care in collaboration with the IDT. The HCC's routine home visits, assessments, and caregiver oversight are performed independently; any finding that requires clinical judgment beyond that scope is escalated to the on-site clinic RN for consultation. The HCC provides delegated home care services for participants and health education support for participants and caregivers, and acts as a liaison between K-Day PACE and contracted home health agencies.

Requirements

Active LVN license in good standing
CPR, AED, and First Aid Certification
Current driver’s license and proof of auto insurance
One year of experience with a frail or elderly population
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Nice To Haves

Minimum of 2 years of skilled nursing experience in a home-based, community, or long-term care setting
Prior experience managing, directly or indirectly, Home Health Aides, CNAs, or caregivers in a home-based, community, or long-term care setting

Responsibilities

Demonstrate effective leadership in managing daily operations of the home care department and services.
Perform initial assessments of participants in their home environment to determine individual needs, assess current support systems, safety of the home environment, and need support services in the home in a timely manner.
Perform scheduled and unscheduled re-assessments to assess home care service needs and provision of services as planned.
Develop a home care plan of care in coordination with the Interdisciplinary Team and caregiving resources in the home (family, friends, etc.).
Participate in the development of the participant’s plan of care at scheduled plan of care meetings.
Maintain regular attendance at, and participate in, interdisciplinary team meetings, communicating participant changes, collaborating care planning decisions, and coordination of 24-hour care delivery.
Provide orientation, initial and annual competencies, supervise, and evaluate performance of caregivers and home care schedulers.
Assure that home care staff provide skilled supportive and restorative nursing care to participants as prescribed in the home care plan of care.
Assure that home care staff maintain participants’ medical records and fulfill charting and reporting requirements.
Assure that home health nursing staff provide education and training to participants and their families on an individual or group basis as needed.
Responsible for the coordination of home care services for all participants in the home.
Serve as a liaison between K-day PACE and contracted home health agencies.
Manage and prioritize utilization of homecare staffing needs to reflect enrollee homecare needs. Provide this information to the interdisciplinary team.
Ensure all contracted home health agency staff complete initial and annual competency testing and fulfill all CMS and K-day PACE training and personnel requirements.
Maintain utilization data for authorized home care services provided.
Participate in developing and organizing new staff orientation, training and on-going in-service training for home care staff and independent contractors.
Provide skilled nursing care in the home when needed.
Participate in the on-call process.
Follow all policies and procedures and OSHA safety guidelines.
Maintain the confidentiality of all company procedures and information about participants, clients or families according to HIPAA regulations.
Handle potentially infectious specimens with appropriate biohazard precautions and practice standard precautions.
Participate in and support Quality Improvement initiatives.
Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
Act only within the scope of his or her authority to practice.
Perform other duties as required or requested.
