K-day PACE is committed to helping older adults remain safely and independently in their homes and communities through comprehensive, coordinated, and culturally attuned care. Our Caregivers are an important part of this mission, providing compassionate in-home personal care and support to K-day participants. The Caregiver provides personal care and assistance with activities of daily living in accordance with each participant’s individualized plan of care, authorized Home Care services, assigned care instructions, and K-day PACE policies and procedures. The Caregiver promotes participant safety, dignity, independence, and quality of life and serves as an important source of observation and communication for the K-day care team. Caregivers promptly report relevant changes in a participant’s condition, functioning, behavior, environment, or care needs to the Home Care Coordinator and/or appropriate clinical team. The Caregiver provides only services for which they have been appropriately trained, assigned, and determined competent and does not independently modify a participant’s plan of care, authorized hours, services, or clinical treatment.
