About The Position

Access Healthcare Associates (AHA) is a mobile medical practice dedicated to serving the geriatric population across assisted living, memory care, board & care, and residential settings throughout Los Angeles. We are seeking a full-time Clinical Supervisor (LVN preferred) to lead clinical operations, oversee departmental performance, and ensure high-quality, coordinated patient care across multiple communities. This is a facility-facing, leadership-driven role focused on managing clinical and scheduling teams, optimizing workflows, and driving accountability across the organization. This position is not direct patient care-focused and requires a candidate who combines strong clinical judgment with managerial leadership, operational oversight, and the ability to create structure, support teams, and maintain consistency across clinical and administrative functions.

Requirements

  • Minimum of 2 years of clinical experience in a healthcare setting, with preference for experience in assisted living or senior care environments.
  • Prior supervisory or leadership experience in a clinical setting strongly preferred.
  • Strong clinical assessment and triage skills.
  • Proficiency with Electronic Health Records (EHR) systems.
  • Excellent organizational, multitasking, and time management skills.
  • Strong communication and interpersonal skills with a professional, compassionate demeanor.
  • Ability to work effectively in a fast-paced, dynamic environment.

Responsibilities

  • Triage patient condition changes (infections, falls, behavioral changes, labs) and ensure appropriate care coordination
  • Oversee care planning, escalation protocols, and transitions of care (hospital/SNF)
  • Ensure timely and appropriate follow-up on all clinical cases
  • Manage Clinical and Scheduling teams, including department leads
  • Drive accountability for performance, productivity, and workflow execution
  • Monitor and enforce timely documentation, follow-ups, and task completion
  • Lead team meetings, huddles, and operational check-ins
  • Serve as a key liaison to assisted living, memory care, and board & care communities
  • Build and maintain strong relationships with facility leadership and staff
  • Address escalations from facilities, families, and internal teams
  • Identify inefficiencies and implement process improvements
  • Develop, refine, and enforce SOPs and standardized workflows
  • Ensure consistency across triage, documentation, and communication processes
  • Support hiring, onboarding, and training of clinical support staff
  • Provide ongoing coaching and performance management
  • Ensure new hires are aligned with AHA workflows and expectations
  • Work Closely with the Director of Nursing (DON) and Medical Director
