About The Position

This role is for a Licensed Vocational Nurse (LVN) at CommonSpirit Health, an organization formed by the alignment of Catholic Health Initiatives (CHI) and Dignity Health. Your daily tasks will include assessing patient conditions, administering medications, performing prescribed treatments, and meticulously documenting care. You will collaborate with Registered Nurses (RNs) and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience. To succeed in this role, you must demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. A compassionate demeanor, attention to detail, and the ability to prioritize in a fast-paced environment are essential.

Requirements

  • Graduate from an accredited Practical/Vocational Nurse program.
  • Licensed Vocational Nurse: TX, upon hire.
  • Basic Life Support - CPR, within 14 days.

Responsibilities

  • Provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.
  • Assess patient conditions.
  • Administer medications.
  • Perform prescribed treatments.
  • Meticulously document care.
  • Collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.
  • Perform general nursing care to patients by following established standards and procedures.
  • Obtain patient consent for care and ensures patient confidentiality.
  • Obtain and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition.
  • Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Execute prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response.
  • Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
