Griswold Home Care is an award-winning, private-pay, non-medical home care agency dedicated to helping seniors and families in Houston remain safe and independent in their homes. This part-time role offers flexible hours and involves a combination of phone work, home visits, and community outreach. You will guide families through stressful times, helping them find the right care, and your nursing background will be valued and respected.

Requirements

Active Texas LVN license
At least 5 years of experience
Reliable vehicle
Valid driver's license
Auto insurance

Home care, home health, or senior living experience
Sales, marketing, or liaison experience
Warm, confident phone manner, comfortable talking with families and professionals
Organized and good at follow-through
Flexibility for occasional evening and weekend client intakes

Responsibilities

Answer and return calls from families inquiring about care.
Listen to family situations, explain services, and schedule/conduct intake visits.
Follow up with prospective clients who have not yet started services.
Maintain up-to-date call notes and follow-ups in the system.
Visit new clients and families in their homes to understand care needs, routines, and preferences.
Assess home safety and assist in matching clients with the right caregiver.
Coordinate care with other healthcare providers.
Document each intake visit.
Serve as a friendly point of contact for families if care needs change as care begins.
Build relationships with local referral sources (e.g., senior living communities, discharge planners, doctors' offices).
Represent Griswold Home Care at occasional community events.
Communicate how services help families, leveraging your nursing expertise.
