About The Position

Vibra Healthcare is seeking a Case Management Extender to join their team at Vibra Hospital of Northern California in Redding, CA. This hospital is a 50-bed Critical Care Hospital and a 32-bed SNF providing long-term acute care services to patients recovering from serious illnesses or injuries. The role offers an opportunity to work in a fast-paced and friendly environment where staff can get to know patients and their families well. The Case Management Extender will participate in a collaborative process to monitor and evaluate options and services to meet patient health needs, ensuring quality, cost-effective care and optimal outcomes. This role is key in ensuring the appropriate sequencing of treatment goals and the implementation of a comprehensive discharge plan, requiring knowledge and skills in case management services.

Requirements

• Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse, Respiratory Therapist, Certified Occupational Therapy Assistant, or a Physical Therapy Assistant in the state of employment required; or a Bachelor’s or Master’s degree in Social Work (BSW or MSW) required.
• Current BLS certification from a Vibra-approved vendor required.
• Proficient in collecting and retrieving material from medical records.
• Ability to project a professional image.
• Knowledge of regulatory standards and compliance requirements.
• Strong organizational, prioritizing and analytical skills.
• Ability to make independent decisions when circumstances warrant.
• Working knowledge of computer and software applications used in job functions.
• Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.

Nice To Haves

• Experience in a rehabilitation or acute care setting preferred.
