About The Position

Vibra Healthcare is seeking a Case Management Extender to join their team at Vibra Hospital of Central Dakotas in Mandan, ND. This is a 41-bed Critical Care Hospital specializing in long-term acute care for patients recovering from serious illnesses or injuries. The hospital environment is described as fast-paced and friendly, offering an opportunity to build close relationships with patients and their families.

Requirements

  • Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse, Respiratory Therapist, Certified Occupational Therapy Assistant, or a Physical Therapy Assistant in the state of employment required; or a Bachelor’s or Master’s degree in Social Work (BSW or MSW) required.
  • Current BLS certification from a Vibra-approved vendor required.
  • Proficient in collecting and retrieving material from medical records.
  • Ability to project a professional image.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing and analytical skills.
  • Ability to make independent decisions when circumstances warrant.
  • Working knowledge of computer and software applications used in job functions.
  • Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.

Nice To Haves

  • Experience in a rehabilitation or acute care setting preferred.

Responsibilities

  • Participating in a collaborative process to monitor and evaluate options and services to meet an individual’s health needs.
  • Providing quality, cost-effective care utilizing selective benefits and alternative services best suited for the patient, while ensuring optimal outcomes, through communication and available resources.
  • Ensuring appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan.
  • Demonstrating knowledge and skills necessary to provide case management services.
