The LTSS Service Coordinator RN Clinician is responsible for the overall management of a member's case within the scope of their licensure. This role involves providing supervision and direction to non-RN clinicians participating in the member's case, adhering to applicable state law and contract. The position requires developing, monitoring, evaluating, and revising the member's care plan to meet their needs, with the ultimate goal of optimizing the member's healthcare across the care continuum. A key responsibility is performing face-to-face clinical assessments for the identification, evaluation, coordination, and management of member needs, encompassing physical health, behavioral health, social services, and long-term services and supports. This is a field-based role, requiring travel to client sites or designated locations, with occasional office attendance for meetings or training. Candidates must reside in Wellington or Wichita Falls, Texas, and be open to travel up to 2 hours daily. Alternate locations may be considered if candidates reside within a commuting distance from an office, but candidates not within a reasonable commuting distance from the posting location(s) will not be considered unless an accommodation is granted.
