This is a hybrid-remote role with 75% local home visits focusing on Physically Disabled/Elderly waiver services. Preference will be given to applicants with past case management, advocacy or home visits/community travel experience who are located in the south and southwest suburbs of Chicago. The position assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. It may involve developing personalized service care plans for long-term care members and educating members and their families/caregivers on available services and benefits. The role evaluates member needs and available resources, recommends and/or facilitates plans for the best outcome, and coordinates between the member, family/caregivers, and the care provider team to ensure services are accessible. It also provides resource support for various member needs, monitors care plans and member status, and interacts with healthcare providers and partners. The role involves collecting and maintaining member information and care management activities for compliance, and may perform on-site visits. Education is provided to members and families on procedures, healthcare provider instructions, service options, referrals, and benefits. Feedback is provided to leadership on improving care quality and service delivery. Performs other duties as assigned and complies with all policies and standards.
