This is a hybrid-remote role with 75% local home visits focusing on Physically Disabled/Elderly waiver services. Preference will be given to applicants with past case management, advocacy or home visits/community travel experience who are located in specific central Chicago neighborhoods. The role assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. It involves developing personalized service care plans, educating members and families on available services, evaluating member needs and resources, and coordinating between members, families, and care providers. The position also provides resource support for various member needs, monitors care plans and outcomes, interacts with healthcare providers, collects and maintains member information, and performs on-site visits. Additionally, it involves providing education to members and families, offering feedback for quality improvement, and performing other assigned duties while complying with all policies and standards.
