This is a hybrid-remote role with 75% local home visits focusing on Physically Disabled/Elderly waiver services. Preference will be given to applicants with past case management, advocacy or home visits/community travel experience who are located in specific Cook, Will, and Kankakee County zip codes near Chicago. The role is within the Long Term Support Services, Case Management department, focusing on Physical Disability & Aging Waiver Members. The schedule is Monday-Friday, 8am-4:30pm with a half hour lunch. The purpose of this position is to assist in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This may involve developing personalized service care plans, educating members and their families/caregivers on available services and benefits, evaluating member needs and available resources, and recommending or facilitating the best plan for outcomes. The role also involves coordinating between members, families/caregivers, and care providers to ensure accessible services, providing resource support for various needs, monitoring care plans and member status, interacting with healthcare providers and partners, collecting and maintaining member information, performing on-site visits, and providing education to members and their families/caregivers. The position also involves providing feedback to leadership on improving care quality and service delivery, and performing other duties as assigned while complying with all policies and standards.
