About The Position

We are seeking a dedicated and experienced LTC Community Team Lead to oversee and coordinate our long-term care community initiatives. The ideal candidate will have strong leadership skills, a passion for improving the quality of life for residents, and the ability to manage a multidisciplinary team effectively.

Requirements

  • Minimum of 3 years experience in long-term care or community health settings.
  • Proven leadership and team management skills.
  • Strong knowledge of long-term care regulations and best practices.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to handle multiple priorities and work effectively in a fast-paced environment.
  • Compassionate and resident-focused approach to care.

Nice To Haves

  • Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or related field preferred.

Responsibilities

  • Lead and manage the LTC community team to ensure high-quality care and services for residents.
  • Coordinate daily operations within the long-term care community, ensuring compliance with regulatory standards.
  • Develop and implement care plans in collaboration with healthcare professionals, residents, and families.
  • Provide training, guidance, and support to team members to promote professional development and teamwork.
  • Monitor and evaluate the effectiveness of care programs and community initiatives, making improvements as needed.
  • Serve as a liaison between residents, families, healthcare providers, and administrative staff.
  • Ensure accurate documentation and reporting in accordance with organizational policies and regulatory requirements.
  • Promote a positive and inclusive community environment that supports resident well-being and engagement.
