About The Position

An LPN in a hospital is a vital member of the patient care team. They work under the supervision of Registered Nurses (RNs) and physicians to provide direct patient care, monitor health conditions, and support patients and families throughout their hospital stay.

Requirements

Completion of an accredited LPN program
Current LPN licensure in the state of practice
Basic Life Support (BLS) certification
Excellent interpersonal, organizational, and problem-solving skills
Strong relationship and communication with physicians, Advanced Practice Providers, and clinical leaders
Continual industry and job specific education
Ability to work in a fast-paced, team-oriented environment
Compliance with all laws, regulatory guidelines, and health care policies (including federal, state, and local laws, State Departments of Health, HFAP, HIPAA, and LCMH Policies and Procedures)
Compliance with hand hygiene guidelines
Adherence to safe practices, and identification and reporting of unsafe practices

Responsibilities

Administer medications and treatments as prescribed
Monitor and record vital signs and symptoms
Assist with diagnostic tests and procedures
Provide wound care and dressing changes
Collaborate with RNs and physicians to develop care plans
Communicate changes in patient condition to the healthcare team
Educate patients and families on care procedures and post-discharge instructions
Maintain accurate and up-to-date medical records
Document interventions and patient progress
Follow infection control protocols
Ensure cleanliness and organization of patient areas
Adhere to hospital policies and legal regulations
Participate in ongoing training and competency evaluations
Contribute to process improvement initiatives
