About The Position
We are seeking compassionate and dedicated Home Care Nurses (RN’s/LPN’s) to join our team to provide exceptional, patient-centered care throughout the Merrimack Valley -or- North Shore region of Massachusetts! The ideal candidate will possess strong clinical, communication, and organizational skills and be committed to delivering exceptional care to patients and their families.
Requirements
- Current Massachusetts LPN license in good standing in Massachusetts.
- Minimum one (1) year of nursing experience.
- Current BLS certification.
- Valid driver’s license and reliable transportation.
- Strong communication, organizational, and documentation skills.
- Must have a criminal background check.
Nice To Haves
- home care experience preferred.
Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies.
- Assess patient needs, monitor clinical status, and document visits accurately and timely.
- Educate patients and families regarding diagnoses, medications, treatments, and care plans.
- Collaborate with physicians, caregivers, and interdisciplinary team members to ensure quality patient outcomes.
- Participate in care planning, case coordination, and ongoing communication regarding patient progress.
- Maintain compliance with agency policies, state regulations, and professional nursing standards.
