About The Position
Great Lakes Management is hiring a Licensed Practical Nurse to support residents in its assisted living community at Amira Choice Arvada, Colorado. The LPN helps maintain residents’ quality of life and a safe, nurturing environment by monitoring clinical needs, coordinating care, overseeing medication practices, and working with residents, families, staff, physicians, and other service providers. The role is scheduled for two days per week, with potential rotating weekends, and pays $32–$38 per hour.
Requirements
- Maintain a current LPN license in the State of Colorado in accordance with state regulations.
- Good organizational skills and the ability to self-start and follow through with initiatives with minimal supervision.
- A caring heart and passion for caring for older adults.
Nice To Haves
- At least one year of experience in home health, assisted living, or long-term care is preferred.
Responsibilities
- Ensure residents continue to meet admission criteria, monitor their needs, and recommend alternative placement when necessary.
- Ensure residents have current care plans in accordance with state regulations.
- Monitor and maintain resident charts and clinical progress notes.
- Communicate changes in residents’ needs or conditions to physicians, staff, and family members.
- Coordinate resident services with other service providers.
- Oversee medications and renewals, including necessary follow-up with physicians about medications or medical conditions.
- Ensure medication administration procedures are followed and discuss necessary corrections or errors with staff.
- Communicate information staff need to know in residents’ logs.
- Provide alternating supervisory visits in coordination with the RN.
- Work collaboratively with all members of residents’ care teams.
- Supervise medication practices and medical equipment used in the building.
- Ensure staff understand and follow resident care procedures.
- Share resident and family concerns with appropriate staff.
- Work with the manager to ensure emergency procedures are in place.
