About The Position

The Admissions Licensed Practical Nurse (LPN) supports the admissions process and facilitates timely access to care by providing a range of pre-admission and admission services. The LPN collaborates with internal and external partners, including hospitals, long-term care (LTC) facilities, and assisted living facilities (ALFs), to resolve barriers to hospice admission. This role provides information and education about hospice services, assists in addressing advanced directive concerns, and ensures a smooth admission process while advocating for patient and family needs. Employees assigned this job title must be willing and able to flex their work location between the various Hospitals and assist with home admissions as needed.

Requirements

  • Current LPN license in the state of Florida
  • Valid Florida driver’s license with an acceptable driving record
  • Graduation from a Licensed Practical Nursing (LPN) program.
  • Medical clearance for communicable diseases prior to patient contact
  • Current immunizations, including but not limited to TB, Hepatitis, and Flu
  • Reliable transportation
  • Level II AHCA (Agency for Healthcare Administration) Background Screening
  • General knowledge of hospice services, philosophy, and admissions procedures
  • Understanding of the Medicaid application process and community resources
  • Familiarity with referral and placement protocols for independent living, assisted living, and long-term care facilities
  • Skilled in empathetic and compassionate communication
  • Ability to identify psychosocial and spiritual needs and recommend appropriate resources
  • Effective collaboration within interdisciplinary teams
  • Cultural competence and cross-cultural sensitivity
  • Proficient in information sharing and delivering sensitive news
  • Strong clinical judgment and decision-making skills
  • Capable of working independently with minimal supervision
  • Flexible and adaptable to change
  • Strong customer service orientation
  • Minimum of two (2) years of experience in the healthcare industry, preferably in acute care, oncology, or long-term care settings.

Responsibilities

  • Provide information to patients, families, and caregivers about hospice services and philosophy
  • Gather data to support and facilitate timely admissions
  • Consult with admissions staff and community partners on advanced directives
  • Document patient and family contacts per organizational policy
  • Build and maintain relationships with social workers, discharge planners, and community agency staff
  • Educate medical professionals on appropriate hospice referrals and the benefits of early referrals
  • Assist with in-service programs and public relations activities
  • Maintain confidentiality in accordance with legal and organizational guidelines
  • Support coordination of care level changes at admission and for hospital-based patients
  • Represent the organization professionally in all interactions
  • Use resources responsibly and promote hospice services in the community
  • Continuously improve professional knowledge and skills
  • Perform other duties as assigned
