About The Position

This role involves directing the day-to-day functions of nursing assistants and ensuring compliance with long-term care facility regulations. The LPN will be responsible for planning shift services, managing resident records, administering medications and treatments, and providing direct patient care. This position requires strong organizational, communication, and leadership skills, as well as the ability to work collaboratively with a healthcare team and provide age-appropriate care to residents.

Requirements

Nursing Degree from an accredited college or university, or be a graduate of an approved LPN program.
Current, unencumbered, active LPN license to practice as an LPN in this state.
BLS / CPR and AED Program Certification is required.
Must be able to read, write, speak, and understand the English language.
Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident’s condition.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Must be able to lift a minimum of 50 pounds and meet all other physical and sensory requirements of the job.

Responsibilities

Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Meet with assigned nursing staff and support personnel to plan shifts’ services, programs, and activities.
Complete and file required recordkeeping forms/charts upon resident admission, transfer, and/or discharge.
Encourage attending physicians to review treatment plans, record and sign orders, progress notes, etc.
Receive telephone orders from physicians and record on the Physicians’ Order Form.
Transcribe physician’s orders to resident charts, cardex, medication cards, treatment/care plans.
Chart nurses’ notes in an informative and descriptive manner reflecting care provided and resident’s response.
Inform nursing personnel of new admissions, their expected time of arrival, and room assignment.
Participate in facility surveys (inspections) made by authorized government agencies.
Perform nursing administrative functions as outlined in the job description.
Thoroughly chart and document by completing and filing required recordkeeping forms/charts upon resident’s admission, transfer and or discharge.
Participate in the orientation of new residents/family members to the facility.
Make rounds with physicians as necessary.
Consult with the resident’s physician in providing resident’s care, treatment, rehabilitation, etc.
Review the resident’s chart for specific treatments, medication orders, diets, etc.
Prepare and administer medications as ordered by the physician, verifying resident identity.
Perform all tasks as an LPN pertaining to Drug Administration tasks.
Ensure adequate supply of floor stock medications, supplies, and equipment. Report needs to the Nurse Supervisor.
Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc.
Collect samples (urine, blood, sputum) for testing and perform routine laboratory tests.
Take and record TPRs, blood pressures, etc.
Check foods brought into the facility by resident’s family/visitors to ensure they are within dietary allowances. Report problem areas to the Nurse Supervisor and Dietary Supervisor.
Provide health care instruction to patient and family members.
Maintain the confidentiality of all resident care information, including protected health information.
Report known or suspected incidents of unauthorized disclosure of information and adhere to Resident’s rights.
Perform other work-related duties as assigned or requested.
