About The Position

The Staff LPN implements, directs, communicates, and documents the nursing process while helping maintain a safe environment for patients, visitors, and associates. The LPN practices according to established standards of care, the Nurse Practice Act, and Powers Health policies and procedures. This position is in Assisted Living at Hartsfield Village, a residential community with nursing presence 24 hours a day, seven days a week, to assist residents with activities of daily living. The position is eligible for a bonus and is scheduled for evenings, 3:00 p.m. to 11:30 p.m.

Requirements

Graduate from an accredited school of practical nursing.
Hold a current LPN license from the State of Indiana Health Professions Bureau.
Obtain and maintain current BLS certification and other required certificates in specialty areas.

Responsibilities

Document a physical assessment each shift, including subjective and objective data.
Communicate with patients, families, and/or significant others to individualize the patient’s plan of care.
Verify that physician’s orders are transcribed properly and carried out promptly.
Report significant changes in a patient’s condition to the unit manager, charge nurse, shift supervisor, and/or physician.
Continuously evaluate patient responses to the plan of care, teaching plans, and rehabilitation activities throughout the stay and upon discharge.
