About The Position
Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Proctor, MN consists 3 cottages/suites and up to 56 residents. We provide the utmost quality of care with an emphasis on person-centered care planning in our home like atmosphere. Are you ready to lead, inspire, and make a difference? Requirements
- Graduate from approved LPN program
- Possess a valid Minnesota LPN License
- Minimum of 1 year experience
- Leadership ability
- Must be able to occasionally fill in aide shifts when needed, this could include weekends.
- Maintaining confidentiality and professionalism at all times
- Must be able to work as a team member and maintain a positive demeanor
- Positive work attitude

Responsibilities
- Medication management & chart auditing
- Performing resident treatments as ordered per the care plan
- Observing, tending to and documenting residents health needs
- Communicating with RN Director regarding relevant staff and resident information and/or concerns
- Reviewing appointments & setting up transportation for residents
- Processing MD orders completely and accurately
- Completing incident reports as needed
- Ensure the staff is performing safe practices
- Executing tours for potential admits
- Reviewing messages and communicating with staff through R-Task
