About The Position

Seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide in-home care for a 21-year-old male with complex medical needs. The client resides with his mother and stepfather and requires assistance with all activities of daily living, including feeding, bathing, grooming, and medication management. He has diagnoses including severe intellectual and developmental disabilities (IDD), spastic quadriplegia, cerebral palsy, neuromuscular scoliosis, epilepsy, and hypertonia of all four extremities. The role involves constant monitoring for seizures, daily suctioning, and administering pureed foods. Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of practice.
• Experience in pediatric or adult home health care.
• Proficiency in medication administration and monitoring.
• Experience with suctioning and managing feeding tubes.
• Ability to monitor for and respond to seizure activity.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a care team.
• Reliable and punctual.

Nice To Haves

• Experience with clients with severe IDD, cerebral palsy, or similar conditions.
• Familiarity with managing neuromuscular scoliosis and epilepsy.
• Bilingual abilities.

Responsibilities

• Provide direct nursing care to a 21-year-old male with complex medical needs.
• Assist with all activities of daily living: feeding, bathing, grooming.
• Monitor and administer medications as prescribed.
• Administer pureed foods and monitor intake.
• Perform daily suctioning.
• Provide constant monitoring for seizures.
• Document care provided and client status.
• Communicate effectively with the client's family and healthcare team.
