About The Position

Three Meadows Post Acute in Perrysburg, Ohio, is hiring a second-shift Licensed Practical Nurse to provide resident-centered nursing care and lead the evening unit. The LPN helps set the tone for the shift by caring for residents, supporting nursing assistants, and keeping unit operations running smoothly.

The role includes hands-on clinical care, communication and coordination with residents’ families and physicians, participation in care planning and quality assurance, and support for residents during admission and discharge. The LPN also promotes safety and infection control practices.

The schedule is consistently 3:00 PM to 11:30 PM. Pay starts at $32 per hour, including shift differential. Requirements

• Graduate of an approved LPN program.
• Current, unencumbered Ohio LPN license in good standing with the State Board of Nursing.
• Current CPR certification.
• Leadership skills and a team-first attitude.
• Patience, tact, and compassion when working with older adults and their families.
• Ability to lift 50 pounds and move comfortably throughout the shift.

Nice To Haves

• PointClickCare (PCC) experience is a plus but is not required.

Responsibilities

• Provide hands-on nursing care, including medication administration, treatments, wound care, and monitoring changes in residents’ conditions.
• Lead the evening shift by supervising and supporting nursing assistants and making daily rounds.
• Coordinate communication among residents, families, and physicians, including physician orders, family updates, and care coordination.
• Work with the care plan team to keep care plans on track and contribute to quality assurance.
• Help new residents with admission and orientation and contribute to discharge planning.
• Promote a safe, infection-free environment and help staff follow safety and infection-control practices.
