About The Position

Enrich the lives of older adults through community, choice, and continuing care in the LPN Team Leader role. As an LPN you will care for ill, injured, or convalescing patients/residents or persons with disabilities in a variety of care settings which may include skilled nursing or long-term care. LPNs may work under the direction of a registered nurse and often direct and supervise the work of nursing assistants who perform direct nursing care of the residents and patients. Requirements

  • A minimum of a high school diploma or equivalent accompanied by experience as a CNA.
  • 2-year college degree in Nursing or related field.
  • Current Washington State Nursing Assistant certification.
  • Active Licensed Practical Nurse (LPN) license in Washington State or out-of-state MSL.
  • Follow the Washington Nurse Practice Act.
  • If MSL, complete Suicide Prevention Training in advance of hire per state regulations.
  • If MSL, Washington State Board of Nursing (WABON) MSL demographic survey.
  • Ability to obtain a Washington State Food Worker Card within 14 days of hire.
  • Ability to obtain a CPR and First Aid certification within 30 days of hire.
  • Effective and comprehendible English verbal and written communication skills to accomplish charting, record keeping, and to comprehend written orders and procedures.
  • Displays a personable and comforting personality to care for the residents.
  • Use logic and reasoning to identify alternative solutions, conclusions, or approaches to problems.

Responsibilities

  • Provide direct nursing care in accordance with resident care plans, provider orders, facility policies, and applicable nursing regulations.
  • Administer prescribed medications or start intravenous fluids.
  • Collect samples, such as blood, urine, or sputum from patients and perform routine laboratory tests on samples.
  • Measure and record patients’ vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
  • Supervise nurses’ aides or assistants and assign the responsibilities for the direct care of a specific group of residents based on the needs of the resident and the needs of the facility.
  • Assure that the nurses’ aides or assistants completes resident care activities in a timely, efficient, and appropriate manner.
  • Assist with the implementation of individualized care plans.
  • Assist with resident admissions, transfers, and discharges as assigned and within scope.
  • Promote infection prevention, resident safety, dignity, privacy, comfort, and quality of life.
  • Care for residents by monitoring health status, feeding, bathing, dressing, grooming, toileting, assisting with mobility, and serving meals and snacks.
  • Establish and interpersonal relationships with residents, families, and coworkers.
  • Document nursing care, observations, treatments, medication administration, and resident responses accurately and timely in the electronic health record.
  • Communicate resident observations and health-related information to others member of the care team including coworkers, supervisors, third-party providers, and families.
  • Learn and apply Company, Federal, State and Local rules, regulations, and policies.
  • Other duties as assigned.
