About The Position

We are seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care for a pediatric patient in a home health setting . In this role, you will work one-on-one with a child in their home environment , delivering personalized care, monitoring health status, and supporting the patient’s medical and developmental needs. This position offers the opportunity to build meaningful relationships with patients and families while providing high-quality, patient-centered care . The hours for this position is Mon-Fri 6a-6p

Requirements

• Current Licensed Practical Nurse (LPN) license in the state or a multistate compact license .
• Current BLS/CPR certification .
• Strong assessment, communication, and documentation skills .
• Ability to provide compassionate care to medically complex children and their families .

Responsibilities

• Provide one-on-one skilled nursing care to a pediatric patient in the home setting.
• Monitor and document vital signs, respiratory status, and overall patient condition .
• Administer medications, treatments, and tube feedings as ordered by the physician.
• Assist with activities of daily living (ADLs) including hygiene, mobility, and comfort measures.
• Observe for changes in the patient’s condition and report concerns to the RN supervisor or physician .
• Maintain accurate and timely documentation according to agency policies and state regulations.
• Educate and support family members and caregivers regarding the patient’s care plan.
• Follow infection control and safety protocols within the home environment.
