About The Position

This position is responsible for providing direct resident care and clinical support within a healthcare setting. The LPN will follow service plans, document resident conditions, communicate with supervisors, and ensure compliance with regulations. They will also be involved in medication administration, staff training, and maintaining a safe and supportive work environment.

Requirements

  • Valid LPN license.
  • Maintain proper certifications required by local and state licensing agencies.
  • On-going in-service trainings, accreditations, specific coursework and certifications.
  • Knowledge of HIPAA, OHSA, and Blood Borne Pathogen Guidelines.
  • Ability to work in a safe manner and promote safety among colleagues.
  • Professional behavior in communication with residents, families, visitors, physicians, and staff.

Responsibilities

  • Follow services outlined in each resident's service plan in a friendly and supportive manner.
  • Document any observed change of condition in accordance with local rules and regulations and policies.
  • Communicate with Director of Health and Wellness on resident needs on an as needed basis to ensure consistent quality services.
  • Provide hands-on care and specific wellness tasks as required and in support of the resident's care plan.
  • Review care plans and ensure the plan meets with resident's specific level of support as required by state regulations.
  • Provide clinical support and training of direct care team in support of continuing education requirements.
  • Assist in transcription of physicians' orders and the review of new physicians' orders.
  • Maintain proper certifications required by local and state licensing agencies, including on-going in-service trainings, accreditations, specific coursework and certifications.
  • Assist the community with compliance with the community quality assurance.
  • Report identified issues not supporting quality services to Executive Director.
  • Provide specific review of Medication Administration Records (MARs) and review of the Daily Report to ensure appropriate clinical interventions are implemented and reviewed on those residents on alert or with acuity changes.
  • Follow all local, state and federal rules and regulations as they apply to services delivered for all residents, including resident care, documentation, and compliance with HIPAA, OHSA, and Blood Borne Pathogen Guidelines.
  • Work in a safe manner, support other employees in working in a safe manner, and correct unsafe actions.
  • Be compliant with Safety Committee Standards.
  • Assist the Executive Director and Director of Health and Wellness with the investigation, return to work, and management of the injured worker communication with care provider and Management Company if workplace injuries occur.
  • Foster teamwork with co-workers, show initiative in overcoming obstacles, take actions that assist others beyond job responsibilities, and have a positive impact on co-workers, residents, and visitors.
  • Model professional behavior in communication with residents, families, visitors, physicians, and both clinical and non-clinical staff in the community.
  • Perform other duties as assigned or needed.
