About The Position

This job involves performing essential duties satisfactorily as an LPN. The role requires adherence to nursing standards and facility policies, effective communication with patients, families, and physicians, and the ability to supervise and evaluate patient care. The LPN will be responsible for obtaining and giving reports, conducting patient rounds, identifying safety hazards, and initiating life-saving measures when necessary. The position also includes participating in facility education programs, providing clinical supervision, attending staff meetings, and managing unit compliance with regulations. Key responsibilities include administering medications and treatments, performing patient assessments, developing and revising care plans, documenting patient care, and coordinating with other departments for admissions and discharges. The LPN will also be involved in supervising care givers, managing narcotics, preparing medications and treatments, supervising restraints, ordering medications, and observing changes in patient conditions. Collaboration with patients, staff, physicians, and external agencies is essential, as is maintaining continuing education and adhering to infection control and safety policies. Dependability and punctuality are required, along with active engagement in quality improvement activities.

Requirements

  • High school diploma or equivalent.
  • Current and good standing LPN license to practice nursing in the State of Idaho.
  • High attention to detail and ability to have critical thinking.
  • Ability to communicate clearly and effectively.
  • Possess the social and verbal ability to understand directions and to be communicative with visitors, patients, and co-workers.
  • Possess the ability to deal tactfully and effectively with patients (and their families), co-workers, and hospital staff.

Responsibilities

  • Interpret functions as set forth in the legal definition of licensed professional nurse.
  • Follow established standards of nursing practices and implement facility policies and procedures.
  • Interpret existing policies and procedures to health care givers, patients, families, and physicians.
  • Supervise and evaluate all direct patient care and initiate corrective action as necessary.
  • Obtain report from the nurse being relieved and record sufficient information for follow-up action.
  • Provide report to the incoming nurse, including sufficient information for follow-up action.
  • Conduct patient rounds daily, reporting problems to nursing supervisor and initiating corrective action.
  • Identify safety hazards and initiate corrective action.
  • Identify patients' problems and emergency situations and initiate immediate "life saving" measures in the absence of a physician.
  • Follow, supervise, and evaluate the implementation of a Patient's Rights Program.
  • Participate in facility education programs.
  • Provide clinical supervision of care givers providing direct patient care.
  • Attend staff meetings, in-service classes, and committee meetings as assigned.
  • Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of patient population.
  • Assume responsibility for compliance with federal, state, and local regulations within the assigned unit.
  • Identify and counsel care givers and recommend disciplinary action to the Nurse Manager.
  • Participate in the employee evaluation process by providing information on the clinical performance of care givers to the Nurse Manager.
  • Complete reports for facility committees as assigned.
  • Administer and document medications and treatments per physician's order and accurately record all care provided.
  • Perform and document comprehensive assessment of each patient.
  • Develop an accurate comprehensive care plan based on each patient's comprehensive assessment.
  • Document patient care provided and patient response or lack of response to care provided.
  • Report changes in patients' conditions to a physician, Nurse Manager, and responsible party, and take follow-up action as necessary.
  • Follow facility admission procedures and communicate patient admission to other departments, physicians, and ancillary service providers.
  • Knowledgeably and completely perform basic nursing skills.
  • Follow facility discharge/transfer procedures and coordinate discharge/transfer with other departments, physicians, and ancillary service providers.
  • Complete documentation related to the patient's discharge/transfer.
  • Assess patient needs and initiate development of individualized care plans.
  • Review and revise patient care plans as necessary and as required by regulations.
  • Participate in patient care conferences as assigned.
  • Provide direct patient care and give instructions to care givers related to patient care.
  • Listen to patient, family, or physician concerns and initiate corrective action and/or report concerns to Nurse Manager.
  • Supervise response to patient calls for assistance.
  • Count all narcotics, sign for count, and exchange keys.
  • Report to care givers and explain all assignments.
  • Check dinner trays to ensure proper diet is served and patients are assisted as necessary.
  • Prepare and administer medications, tube feedings, and treatments as ordered.
  • Supervise application and removal of all postural supports and restraints.
  • Order medications as needed, according to facility procedures.
  • Supervise all patient care on assigned unit.
  • Observe, evaluate, and document all changes in patient's conditions.
  • Notify physician, when necessary, of changes in patients' conditions, weight changes, medication errors, medication not available, abnormal lab and x-ray results, and any other information as necessary.
  • Administer PRN medications and treatments as necessary.
  • Participate in resident care conferences as scheduled.
  • Consistently work cooperatively with patients, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
  • Personally obtain continuing education related to the health care field of nursing.
  • Observe infection control procedures.
  • Observe all facility safety policies and procedures.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Assume accountability for data contained in the Personnel Policy Manual.
  • Perform other related duties as directed by the Nurse Manager.
  • Check and stock the necessary supplies and equipment.
  • Meet attached physical and environmental requirements as indicated.
  • Actively engage in quality improvement activities.
  • Perform other duties as assigned.
