About The Position

LifeStream, Inc. is seeking full and part-time Licensed Practical Nurses (LPNs) to ensure the medical needs of individuals with disabilities are met. In addition to medical care, LPNs will assist individuals with daily living skills, including emotional and social support, in their homes and in the community. This role offers the opportunity to make a direct impact on the lives of individuals with disabilities.

Requirements

• Current LPN license
• Valid driver’s license
• Access to a vehicle
• Ability to pass our employment screening process
