About The Position

Andwell Health Partners is seeking a compassionate and dependable Hospice LPN to provide exceptional, person-centered care in the community. This role focuses on making a meaningful difference for patients and families during life's important transitions, emphasizing care that is about more than just treating a diagnosis. The LPN will provide skilled nursing care, monitor patient conditions, administer medications, educate and support patients and families, and work collaboratively with an interdisciplinary team to ensure patients remain comfortable and supported while honoring their goals and preferences. Accurate clinical documentation and adherence to hospice standards of care are essential.

Requirements

  • Current LPN license in Maine.
  • Strong clinical skills and sound judgment.
  • Excellent communication and organizational skills.
  • A compassionate, respectful approach to patients and families.
  • Ability to work independently while being an engaged member of a collaborative team.

Nice To Haves

  • Hospice, home health, or palliative care experience is valued.
  • Passionate about learning hospice care.

Responsibilities

  • Provide skilled nursing care to hospice patients in their homes and other care settings.
  • Monitor patient conditions and communicate changes to the hospice care team.
  • Administer medications and treatments as directed by the plan of care.
  • Educate and support patients and families with compassion, patience, and respect.
  • Work collaboratively with RNs, providers, social workers, chaplains, aides, and other members of the interdisciplinary team.
  • Help patients remain comfortable and supported while honoring their goals and preferences.
  • Complete accurate clinical documentation and follow hospice standards of care.
