This position involves conducting daily nursing functions in accordance with current rules, regulations, and guidelines governing long-term care facilities and assisted living communities. The role requires ensuring that all assigned nursing personnel comply with written policies and procedures, including the safety program. Responsibilities include admitting, transferring, and discharging residents with appropriate documentation, participating in resident and family orientation, and making rounds with healthcare providers and Hospice/Home Health staff as needed. The LPN will utilize nursing personnel qualifications according to resident care plans, service plans, and healthcare provider orders. Communication and documentation are key, involving daily rounds to evaluate care, resolving resident and family issues, reporting significant events to supervisors, and participating in facility surveys. The role may also involve responsibility for clinical programs such as dementia, wound care, quality assurance, infection control, and admissions. Initial assessments and the development/updating of care plans and service plans are also part of the duties. Effective communication with healthcare providers, pharmacies, and nursing management is essential, along with accurate and timely documentation, including accident/incident reports. The LPN will also ensure rooms are ready for new admissions, greet new residents, notify practitioners and families of condition changes, and assist with arranging transportation and other services. Medication administration includes preparing and administering medications, communicating with the pharmacy, ensuring adequate supplies, and reviewing medication administration records for accuracy. Personnel functions involve supervising the nursing unit, addressing staff performance issues, and reporting complaints and grievances. Safety and sanitation include assuring protective equipment availability, maintaining clean work areas, recommending equipment and supply needs, and ensuring adequate stock levels. The role may involve working nights, weekends, and holidays.
