About The Position

This role involves coordinating the nursing care of residents on a unit, ensuring appropriate assignments for nurse aides, and providing safe and accurate medication administration and treatments. The position also includes significant staffing responsibilities, such as maintaining employee contact information, creating monthly nursing schedules, coordinating modified work duty employees, and addressing staffing variances. The LPN will also be responsible for preparing work assignment sheets, reporting staff shortages, and participating in employee performance discussions. Additionally, the role requires being on call and working every other weekend.

Requirements

Must have a current state license to practice as a licensed practical nurse (LPN).
Current CPR certification.
Demonstrates knowledge of basic nursing practice with an interest in gerontological nursing.
Basic computer skills.
Must be able to be on call and work every other weekend.

Nice To Haves

Additional certification in nursing specialty desired.
Experience with Electronic Medical Records Preferred.
Prefer 2 years’ experience in a long-term care setting.

Responsibilities

Coordinates the nursing care of residents on the unit, assuring all residents are assigned a nurse aide capable of providing care.
Uses independent judgment and discretion on behalf of the organization in the performance of duties.
Provides safe and accurate medication-related interventions to residents.
Administers and documents medications and treatments according to each resident’s medication schedule using current standards of medication pass technique.
Knows the rationale for the effect of medications and treatments and correctly administers same.
Provides treatments per the physician order including wound care, urinary catheterization, oxygen care, tracheostomy care, etc.
Orders and receives medications from the pharmacy.
Conducts an accurate accounting of all narcotics, signing for count and exchange of keys to medication carts and medication room.
Promptly responds to resident requests for pain intervention and documents such in the resident record.
Accurately transcribes and coordinates the implementation of physician orders.
Performs nursing assessments regarding the health status of residents.
Notifies the physician of changes in status and promptly implements and documents new orders.
Prepares work assignment sheets, taking into consideration resident safety, and the educational preparation, experience, knowledge, and ability of the persons to whom the assignments are made.
Reports staff shortages to nursing management.
Participates and communicates employee actions that may result in reward or discipline.
Maintain a current listing of all Nursing employee phone numbers.
Work with Director of Nursing when scheduling modified work duty employees in accordance with work restrictions and facility policy.
Complete monthly nursing schedule coordinating requests to ensure appropriate coverage of units.
Complete and post work sheets/time schedules. Coordinate variances with the Director of Nursing.
Answer employee calls regarding staffing/scheduling functions.
Prepare written correspondence as necessary.
Perform miscellaneous duties pertaining to staffing and assist nursing supervisory personnel as necessary.
Consult with nursing department staff and supervisors concerning the staffing/scheduling needs to assist in elimination/correction of problem areas, and/or improvement of services.
