About The Position

As an LPN Visit Nurse with Home Health Care, Inc., your workday takes you into the community, providing skilled nursing care to patients where they actually live. You drive to your patient. Walk through the door. Focus on one person at a time. Provide the care they need. Then head to your next visit. Your office just happens to have a windshield. Home health gives you something that's difficult to find in a traditional facility: Space to focus. Instead of trying to divide your attention between an entire hallway of patients, you'll provide scheduled skilled nursing visits throughout your local territory. You'll assess. Treat. Educate. Listen. Document. And then move on to your next patient. There's independence—but you're never on your own. You'll have experienced RN Case Managers, Clinical Managers, physicians, therapists, and interdisciplinary clinicians working alongside you to coordinate each patient's care. Your patient visits will be scheduled within approximately a 35-mile radius of your home, allowing you to serve patients in communities closer to where you already live. That means your home location helps shape the territory you serve.

Requirements

  • Active Minnesota Licensed Practical Nurse (LPN) license
  • Strong clinical and patient-care skills
  • Confidence working independently in patients' homes
  • Excellent communication and organization
  • Compassionate, dependable, patient-centered approach
  • Reliable vehicle, valid driver's license, and current auto insurance
  • Ability to travel to patients within approximately a 35-mile radius of your home

Nice To Haves

  • Home health experience preferred, but not required
  • Experience from environments such as: Skilled nursing, TCU, Rehabilitation, Assisted living, Hospice, Clinic, Other direct patient-care settings can provide a strong clinical foundation for home health nursing.

Responsibilities

  • Skilled nursing assessments and observations
  • Wound care and dressing changes
  • Medication management and education
  • Injections and other ordered nursing treatments
  • Monitoring chronic conditions and changes in health status
  • Reinforcing physician orders and treatment plans
  • Teaching patients and caregivers about medications, diagnoses, and disease management
  • Identifying concerns and communicating changes to the RN Case Manager or physician
  • Collaborating with therapists and other interdisciplinary team members
  • Maintaining accurate and timely clinical documentation
  • Help patients continue receiving the skilled nursing care they need without leaving home.
