About The Position
WesleyLife Home Health provides comprehensive home health care throughout central Iowa. The LPN Visit Nurse delivers individualized nursing care in clients’ homes to help them remain safe and independent, monitor health and well-being, and support continuity and quality of care. The role includes documenting visits in an electronic medical record, participating in nursing case conferences and team meetings, and completing ongoing training. The position includes rotating on-call coverage, including evenings, weekends, and holidays. The advertised opening is full-time, Monday through Friday, with rotating weekend on-call. WesleyLife uses AI auto-charting to help practitioners save time with charting. Candidates must complete a pre-hire health assessment and four-panel drug screen. WesleyLife cannot sponsor work visas.
Responsibilities
- Provide hands-on nursing care according to individualized care plans.
- Monitor changes in clients’ health and well-being and promptly report updates to the care team.
- Document visits accurately and clearly in the electronic medical record (EMR).
- Participate in on-call rotations, including evenings, weekends, and holidays, and provide urgent support.
- Participate in nursing case conferences and team meetings.
- Complete ongoing training.
