WesleyLife at Home is hiring a Visit Nurse (RN) to provide personalized home health care to older adults across central Iowa. The role supports clients’ health, safety, independence, and well-being in their homes. Care is tailored to individual needs and coordinated with an interdisciplinary team. The organization uses AI Auto-Charting technology within its EMR to reduce manual charting. Candidates must complete a pre-hire health assessment and 4-panel drug screen. Requirements
- Registered Nurse (RN).
- Experience in at least one of the following areas: home health, wound care, skilled nursing, infusion therapy, or catheter care.
- Complete a pre-hire health assessment and 4-panel drug screen.
Responsibilities
- Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in-home visits.
- Evaluate client progress, response to treatment, and ongoing needs to ensure person-centered care.
- Counsel clients and families on health conditions, care plans, medication management, and related needs.
- Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
- Communicate with the Nurse Case Manager about healthcare provider order updates, changes in client condition, incident reports, and other relevant matters.
- Collaborate with the interdisciplinary team to ensure coordinated care.
