LPN Visit Nurse | One-on-One Home Care
About The Position
Home Health Care, Inc. is seeking an LPN Visit Nurse to provide skilled nursing care to patients in their homes within the Twin Cities Metro Area. This role offers a unique opportunity to move away from the traditional facility setting and focus on one-on-one patient care in the community. As an LPN Visit Nurse, you will have the independence to manage your schedule and territory, which will be approximately a 35-mile radius from your home. You will work alongside experienced RN Case Managers and an interdisciplinary team to coordinate patient care, assess patients, administer treatments, educate patients and caregivers, and maintain accurate documentation. This position allows you to make a direct impact on patients' ability to remain safely at home.
Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license
- Strong clinical and patient-care skills
- Confidence working independently in patients' homes
- Excellent communication and organization
- Compassionate, dependable, patient-centered approach
- Reliable vehicle, valid driver's license, and current auto insurance
- Ability to travel to patients within approximately a 35-mile radius of your home

- Home health experience preferred
Responsibilities
- Provide skilled nursing assessments and observations.
- Perform wound care and dressing changes.
- Manage and educate patients on medications.
- Administer injections and other ordered nursing treatments.
- Monitor chronic conditions and changes in health status.
- Reinforce physician orders and treatment plans.
- Teach patients and caregivers about medications, diagnoses, and disease management.
- Identify concerns and communicate changes to the RN Case Manager or physician.
- Collaborate with therapists and other interdisciplinary team members.
- Maintain accurate and timely clinical documentation.
- Help patients continue receiving skilled nursing care without leaving home.
