About The Position
Garden Gate Health Care Facility is seeking an experienced LPN Unit Coordinator to support daily clinical operations and provide leadership on the unit. This role is ideal for an LPN who enjoys coordinating care, supporting staff, and ensuring high-quality resident outcomes.
Requirements
- Active LPN license
- Strong organization and communication skills

- Prior SNF/LTC or charge experience preferred
