About The Position
The Team Facilitator serves as a key liaison between clinical and operational teams, supporting the efficient delivery of home health services across the region. This role is responsible for coordinating critical workflows, maintaining the accuracy and compliance of electronic medical records, managing physician orders and clinical documentation, and supporting communication between field clinicians, physicians, coding teams, and office staff. The Team Facilitator helps ensure regulatory compliance, operational efficiency, and continuity of patient care while providing administrative and clinical support to home health teams. Current Connecticut LPN licensure is required.
Requirements
- Current Connecticut LPN license required.
- Minimum 1 year of healthcare experience required.
- Strong organizational, communication, and documentation skills.
Nice To Haves
- Home care experience preferred.
- Experience with electronic medical records and Microsoft Office applications preferred.
Responsibilities
- Coordinate and oversee the Resumption of Care (ROC) process for transitioning patients back home.
- Monitor coding queries and facilitate timely, accurate resolution.
- Manage wound care orders, documentation, and communication with care teams.
- Review and route physician orders to ensure accuracy and compliance.
- Support SBAR communications, medication reconciliation, and care coordination activities.
- Maintain accurate documentation within electronic medical record systems.
- Monitor inventory and coordinate ordering of clinical and office supplies.
- Collaborate with clinicians, physicians' offices, coding teams, and operational leadership to support quality patient care and workflow efficiency.
