About The Position

As a Licensed Practical Nurse (LPN) you will be an integral part of our nursing team, responsible for delivering direct patient care under the supervision of Registered Nurses (RNs) and Nurse Managers. Your role will include administering medications, providing treatments, and collaborating with the healthcare team to ensure the well-being and comfort of our patients. This position requires clinical expertise, strong communication skills, and a commitment to delivering patient-centered care.

Requirements

  • Previous experience as an LPN in a healthcare setting is preferred.
  • Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Current Licensed Practical Nurse (LPN) license in the state of Illinois.

Responsibilities

  • Conduct initial patient assessments and gather relevant health information, including vital signs, medical history, and current condition.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate dosages, routes, and documentation.
  • Provide wound care and dressings, monitor healing progress, and report any concerns to the healthcare team.
  • Start and maintain intravenous (IV) lines, administer IV medications, and monitor IV fluid therapy.
  • Educate patients and their families on healthcare procedures, disease management, and medication instructions.
  • Collaborate with RNs and physicians to develop and implement patient care plans, ensuring they meet the individual needs of each patient.
  • Maintain accurate and detailed patient records, including nursing assessments, care plans, progress notes, and medication administration records.
  • Ensure patients are comfortable and address their physical and emotional needs, providing support and companionship.
  • Assist patients with mobility and positioning, including transferring in and out of beds or chairs and providing assistance with ambulation.
  • Follow infection control protocols, maintain a clean and safe patient environment, and assist with patient safety measures
  • Work closely with RNs, physicians, and other healthcare professionals to ensure a coordinated approach to patient care.
  • Respond to medical emergencies and provide basic life support as needed.
