About The Position
The Visiting Nurse Association of Florida (VNA), a 50-year-old home health agency, is expanding and seeking Licensed Practical Nurses (LPNs) for home health care visits in Citrus County. In this role, the LPN will collaborate with healthcare referral sources to deliver essential services to individuals and families within their homes. Key responsibilities include conducting physical assessments to identify patient home care needs and assisting in the development and implementation of individualized care plans, adhering to VNA of Florida's case management model. Responsibilities
