About The Position
The Visiting Nurse Association of Florida (VNA), a 50-year-old home health agency, is expanding and seeking to hire Licensed Practical Nurses (LPNs) in Marion County. In this role, LPNs will collaborate with healthcare community referral sources to deliver services that address the needs of individuals and families within their homes. Key responsibilities include conducting physical assessments, identifying patient home care requirements, and contributing to the development and implementation of patient care plans, utilizing VNA of Florida's established case management model. Requirements
- Hold a current Licensed Practical Nurse (LPN) license
Responsibilities
- Work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes
- Complete physical assessments and determine any patient home care needs
- Assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model
