This position is hybrid, with half the hours worked in the company/office and the other half done remotely. The Nurse will provide direct services to clients by implementing the SP/SPA, documenting general health and vitals, and providing general health recommendations to the Designated Coordinator. The role involves administering injection medications, accumulating and documenting individual progress on a regular basis (monthly, bi-weekly, or weekly depending on needs), and providing training, counseling, and assistance in routine healthcare. The Nurse is responsible for the health, safety, and welfare of each individual, adhering to company policies and procedures, and remaining available for hours dictated by individual needs, including being on-call for medical emergencies. Nursing visits should be scheduled with the home prior to arrival. The position reports to the Designated Manager and requires timely submission of timesheets and completion of nursing consult sheets. The Nurse will receive and develop plans to address recommendations, concerns, and feedback from home supervisors, the Designated Manager, and individual teams. Collaboration with the Designated Manager is expected for formulating training that benefits individual programming and medical care. Responsibilities also include managing medical administrative paperwork such as Medication Administration Records, Psychotropic Requirements, Med Referral Sheets, and Health Action Plans, as well as receiving, following up on, and reporting medication errors and medical referrals/doctor's orders. Medical training may also be required.
