About The Position

The Visiting Nurse Association of Florida (VNA), a 50-year-old home health agency, is seeking Licensed Practical Nurses (LPN) or Registered Nurses (RN) in Hobe Sound. In this role, you will collaborate with healthcare referral sources to deliver services that cater to the needs of individuals and families within their homes. Key responsibilities include conducting physical assessments to identify patient home care requirements and contributing to the development and implementation of patient care plans, adhering to VNA of Florida's case management model. The agency emphasizes a supportive environment with flexible scheduling and a variety of client cases.

Requirements

• Must have Trach/Vent and GTube/ Supra Pubic experience
• Must have a minimum of 1-year Clinical Experience
• Monday, Tuesday and Thursday and some weekend Availability

Responsibilities

• Work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes
• Complete physical assessments and determine any patient home care needs
• Assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model
• Provide treatment and care to patients in their home
