About The Position

Phoenix Home Care & Hospice is seeking compassionate RN/LPN Private Duty Nurses to provide one-on-one care to children and young adults in their homes. We understand the demands on nurses and offer flexible scheduling and a supportive team environment to help you find balance. You'll have the opportunity to care for one patient at a time, build lasting relationships through consistent schedules, and make a meaningful difference in patients' and families' lives.

Requirements

• Active RN or LPN license
• Strong assessment and communication skills
• A compassionate heart, dependable character, and self-motivated spirit
• Valid driver's license and reliable transportation
• Eligible to work in the United States

Nice To Haves

• Experience working independently in a one-on-one home care setting is a plus.
• Experience administering medications and monitoring changes in patient condition is a plus.
• Experience with clinical observation and documentation is a plus.
• Experience supporting children, young adults, and families in a care setting is a plus.

Responsibilities

• Provide compassionate, one-on-one nursing care in the comfort of your patients' homes.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate, encourage, and support family members.
• Work alongside the Phoenix clinical team to provide exceptional care.
