About The Position
Angels of Care Pediatric Home Health is seeking compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering patient-centered care according to physician orders and active plans of care, ensuring positive patient outcomes through ongoing assessments, medication administration, treatment, and family education. Requirements
- Active RN or LPN/LVN license
- Provide care in a client home setting

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
