LPN / RN Home Health Nurse
About The Position
Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in the home setting for infants, children, and/or adolescents. The role focuses on the medical needs and treatment according to the physician's plan of care, including ongoing patient assessments, administration of medication, treatments, and therapies. Responsibilities also include coordinating care, educating family members, and implementing preventative initiatives to ensure quality patient outcomes. The position offers full-time and part-time shifts, with days, nights, and weekends available.
Requirements
- Active RN or LPN/LVN license
- Provide care in a client home setting
- Ability to make a positive and lasting impression!

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care
- Educate family members on patient clinical care to enhance positive outcomes
- Preventative initiatives to protect quality of care for patient
